One looks into the arachnoid cyst onto the basal cisterns. The optic nerve and the internal carotid artery can be seen shimmering through the arachnoid mater. Across the top of the screen we see the middle wing of the sphenoid and the sphenoid sinus.The surgical treatment of a symptomatic arachnoid cyst is performed by a microsurgical opening of the cyst into the basal cisterns, the subarachnoid space or the ventricular syste. Thus a further increase in the size of the cyst is prevented. Usually the opening is made between optic nerve and internal carotid artery (magnified). Upon successful operation pulsating cerebrospinal fluid can be seen.
