An inflammation of the nail bed was first seen during basic nursing. This was treated conservatively with Betadine ointment. However, there was no significant improvement, so that the patient made an appointment with the family doctor. The doctor then referred the patient to a surgeon who initially continued the treatment like before. After 6 weeks the doctor noticed a significant aggravation -> vascular bypass (Y-graft) in the thigh. After this did not improve the blood supply as planned, the patient refused further surgical intervention. After informing the patient through the nursing service, the family doctor and the vascular surgeon over the rfurther course, the patient stated he wanted to "die". During the course I initially carried out a palliative wound care daily, later every second day.
