Improper laparoscopic nephrectomy: During the nephrectomy, a double J catheter was inadvertently cut. This remained unnoticed and the remaining part of the catheter was left inside the ureter. The operator, who was a non-urological surgeon, wasn't aware of the presence of the catheter. The remaining piece had to be removed 12 months later because of persistent dysuria.
