56-year-old patient who came into our practise for the first time. Extraoral examination showed a swelling of the right cheek. Suspected was an acute abscess. During the intraoral examination an enormous calculus was found. The anxious patient gives no information about the last time he visited a dentist. He requested to be treated under general anesthesia.
