This patient has skin cancer with deep infiltration at a cosmetically and functionally very unfortunate site. Nasal surgery with normal removal of the nasal ala and direct wound closure would cause asymmetry of the nose and impediment of nasal breathing.
Surgery preparation: To close the created defect a skin flap from the philtrum is swiveled up. The incision lines are drawn onto the skin.
