The patient suffers from a stage 4 decubitus with necrosis of the Achilles tendon. The picture shows the condition after debridement and VAC therapy.
The therapy with hypotension is supposed to cause a good adaptation of the wound margins, remove exudates and infectious material, and reduce edema. More fibroblasts immigrate, which supports the growth of granulation tissue.
When a decubitus ulcer is that deep and widespread, the surgical treatment (debridement, VAC therapy, maybe further - also plastic - surgery) is very difficult and can take months.
