Vein of Galen malformations (VGM's) are uncommon, but present dramatically during early childhood, and continue to be challenging to manage. CTA of the brain of an infant demonstrates marked dilatation of a vessel in the region of the vein of Galen with multiple enlarged feeding arteries from both sides. Drainage is via both a persistent falcine sinus and the straight sinus. (Note it is not actually the vein of Galen, but rather a persistent median prosencephalic vein).
