The patient came with a phlegmon of the abdominal wall. This was the result of outpatient surgery in a surgical practice for treatment of an hernia. You can see the almost complete necrosis of the umbilicus. The only option here is to resect the umbilicus, this procedure is called omphalectomy, followed by vacuum wound treatment. Finally, the wound can be closed with a secondary suture.
