Diagnosis: 51 year old patient, concentric myocardial hypertrophy, focus on the left side, subacute necrosis
Description: Massive thickening of the left and some of the right ventricle. Subacute necrosis area on the lateral side.
Additional finding:
Rheumatoid arthritis and non-classified vasculitis of the small and middle sized vessels, multiple organs affected. Restrictive pneumopathy after therapy with methotrexat. Abscess- forming pneumonia through aspergillus. Acute diffuse lesions of the alveolar area.
Clinical presentation: Hypertensive and valvular cardiomyopathy. 7 years ago primary diagnosis of a rheumatoid arthritis and steroid therapy. 7 months ago diagnosis of a non-classified vasculitis, multiple organs affected. Steroid therapy. Cavernous aspergillosis of the lung. No respiratory decompensation after antifungal therapy. Reduction of the steroid dose followed. Failure of the kidneys.
